The Physiological Basis of a Nightfall

Nocturnal emission is a type of spontaneous orgasm, involving ejaculation during sleep. This is an autonomous reflex mediated by the sympathetic nervous system. It usually starts after puberty when there are important hormonal changes occurring in the body especially the increase of testosterone. This results in the growth of sexual organs and the process of semen production. Sometimes the semen produced leaks out of the reproductive organ system during REM sleep when there is a physiological erection resulting in nocturnal emission of semen or nightfall. Usually, it is noticed around the age of 13-15 years but with earlier age of onset of puberty worldwide, the mean age of the first nocturnal emission is decreasing, even before 12 years. In addition to the global earlier onset of puberty, easier accessibility of visual erotic material including television, magazines and the Internet might be responsible for this early onset of nocturnal emission. This problem of wet dreams also occurs in girls, though it is rare. The frequency of experiencing nightfall in men usually decreases with age after 35 years of age. Is it Normal or Abnormal??

The treatment is based on determining the cause of the problem and then treating it accordingly. There is considerable variation among men in the frequencies with which they have nocturnal emissions (71 to 94%), with the highest frequency occurring in the late teens. According to a study from AIIMS in India, it happened in 713 patients out of 1000 consecutive patients between 21 and 30 years of age with sexual disorders attending the psychosexual clinic (71.3%). Another study from Indonesia found that 82.7 percent of young males had a nocturnal emission. So, it is clear that not all will have this process happening to them. Men who have never experienced a nocturnal emission vary from 6 to 29%. It can occur with or without dreams. Almost 37.5% of men have experienced nocturnal emissions without awareness of dreaming.

Composition of Nocturnal Emission And Nightfall

The mean semen volumes, sperm concentrations, sperm motility and sperm morphology of the nocturnal emission were lower than semen obtained by the normal ejaculatory process in a study, but still significant sperm count and semen volume are present. So, it can be deduced that problem of frequent nightfall must be addressed on similar lines to the loss of semen through other means.

Studies have shown that those males whose minds are preoccupied with sexual thoughts, watch porn and masturbate have more frequency of night falls. Anything that makes semen watery also makes it more likely for nightfall to occur. Masturbation, hot, spicy foods and hot beverages especially at night make semen watery in consistency. A full urinary bladder also puts pressure on seminal vesicles increasing the chances of nightfall. 

Anxiety, stress, panic disorder and depression also make you prone to nightfall. Panic and anxiety disorders could be linked to sympathetic overactivity including increased dopamine and norepinephrine activity, in addition to reduced serotonergic activity. Nocturnal emission appears to be partially controlled by testosterone; in this instance, testosterone booster drugs and injections increase the sex drive and the chances of nightfall dramatically due to hormonal changes. Subjects watching TV more than 3 h/day were found to have more nocturnal emissions than the ones watching less. Psychiatric medicines like anti-depressants and anti-psychotics can also result in spontaneous nocturnal emissions. Pathological cause includes spinal cord injury possibly due to loss of inhibition on ejaculation from descending inhibitory pathway.

The Harmful Effects of Excessive Nightfall?

Occurrence up to 2-3 times in a month may be considered physiological but excessive nightfall happening more than 2-3 times in a week should be viewed seriously. Dizziness, spinning sensation, generalised weakness, lethargy, loss of glow on face, back pain, knee pain, feeling low, depression, and lack of sleep are some of the symptoms one experiences with frequent nightfall. Males with almost daily nightfall also reported poor sexual function like erectile dysfunction, premature ejaculation, infertility, and loss of sight and memory. Urine may also accompany the sperm in a few cases. There is no increased risk of prostatic cancer with nocturnal emissions as found in other studies.

Treatment Options For Nightfall
